Role Description Belle is seeking a compassionate and clinically skilled Nurse Practitioner (NP) to provide remote telehealth support for Medicare beneficiaries enrolled in our Chronic Care Management (CCM) program. This role focuses on responding to urgent clinical escalations from our nursing team, with the majority of calls involving poorly controlled hypertension, medication concerns, and other non-life-threatening acute issues. As a Telehealth Provider, you will: - Assess members remotely. - Determine the appropriate level of care. - Provide evidence-based treatment recommendations. - Collaborate closely with nurses, Community Health Workers, primary care providers, and care coordinators.
