• Support day ‑ to ‑ day implementation of Career Pathways initiatives across both countries, including coordinating workplans, tracking milestones, preparing materials, and supporting delivery of pilots, convenings, and partner engagements under the direction of the Implementation Leads • Assist with operational execution of initiatives by coordinating tasks across matrixed teams to ensure timelines, dependencies, and deliverables remain aligned • Support Implementation Leads in managing relationships with ecosystem partners by providing logistical and administrative coordination to ensure smooth execution of multi-stakeholder projects across the U.S. and Canada • Contribute to cross‑country cross-pollination by surfacing comparative work, opportunities for shared partner engagements or connections where initiatives or learnings overlap • Capture implementation‑level insights, risks, and learnings; maintain organized documentation and feed synthesized insights into the Deputy Director, Insights Manager, and Head
Job Requirements
- Bachelor’s degree or an acceptable equivalent combination of education and experience in a relevant field
- 3+ years of related professional experience coordinating administrative activities and/or events; preference for experience in workforce development, immigration, refugee resettlement or economic development
- Lived and/or learned insight into the opportunities and challenges experienced by immigrant and refugee communities
- Experience organizing events from planning to execution
- Exceptional verbal and written communication skills
- Strong strategic thinking, with excellent planning, problem solving, and analytical skills
- Ability to capture and document learnings and synthesize information
- Poise, confidence, and maturity in interacting with key stakeholders and handling sensitive information as required
- Highly organized and detail-oriented in planning and executing tasks
- Strong command of technology tools, including Microsoft Office suite (especially Word, Excel, PowerPoint, SharePoint, and OneDrive), Salesforce or other CRM systems
- Experience using shared systems and platforms (e.g., Monday.com)

• Serve as a liaison between health and social services and the community • Identify health-related issues, collect data, and discuss concerns with the people served • Address members housing concerns and connect them with community resources • Help with completion of applications, identifying housing vacancies, adding members to wait lists • Assist members in understanding and utilizing their housing benefits in partnership with the community and policies related to the Medicaid population • Conduct in-person assessments to understand member care needs, preferences, socioeconomic barriers, and evaluate the home environment • Coordinate access to basic needs with a focus on housing (e.g., housing, food, income, and transportation) • Provide social support to boost members' morale and sense of self-worth • Support member independence through appropriate education and coaching • Conduct research and in-person outreach to locate difficult-to-contact members to increase assessment completion and participation in clinical programs • Build and maintain relationships with providers and community resources to support member referrals • Regularly travel to conduct member visits and community-based visits as needed to ensure effective program administration
• Screen and coordinate patient admissions and transfers from hospitals and clinics • Use clinical decision-making and approved criteria to ensure the proper level of care • Collaborate with intake teams, physicians, nursing staff, and hospital departments to facilitate timely and appropriate placements • Monitor patient flow, track delays, and help improve access processes for better outcomes • Serve as a key liaison between facilities, patients, families, and providers — ensuring compassionate and efficient service • Navigating referrals submission to final acceptance in critical time (45 minutes) • Ensure compliance with EMTALA, Lifepoint Health policies, and quality standards. • Demonstrates strong clinical judgment, critical thinking, and the ability to multitask in a high-volume setting typing 30-40 wpm
• Accurately read and measure student performance in Spanish and French based against a grading rubric • Collaboratively work with the Lead for the course(s) to provide the student with constructive, supportive and concise feedback • Complete grading rubrics and leave detailed feedback and comments for the learner • Check student submissions for plagiarism and for grammar • Provide in-context feedback and scoring using the Sophia-provided rubric • Participate in calibration efforts to ensure consistency of quality for our learners
