Job Description Working closely with the population health teams, primary care providers (PCP’s), networked facility partners, and practice operations at Privia Health, the Health Advisor will provide care coordination and support for Privia’s at-risk patients. They will provide inbound and outbound telephonic support for care management team. Primary work will include outbound calls to patients to facilitate transitions of care, provide coordination services, fill gaps in care, and promote improvements to patient outcomes. This role is also eligible for an annual bonus targeted at 10% .The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
