This candidate must have relevant clinical experience and/or be a certified Emergency Medical Technician (EMT) or Medical Assistant (MA). - Conduct timely post-discharge follow-up calls with patients after inpatient visits to assess needs and reinforce the plan of care. - Review and reconcile discharge medications and escalate any concerns to the Primary Care Provider. - Assist in coordinating and/or scheduling follow-up appointments with the patient's Primary Care Provider and/or specialists - Identify high risk patients and refer to the appropriate Care Management team. - Review Emergency Department discharges and facilitate appropriate follow-up care - Identify and address patient's Social Determinants of Care needs by connecting with appropriate resources - Assist in review and completion of patient's open quality gaps. - Demonstrate strong foundational medical knowledge, including common disease states, diagnoses, medications, care plans, and the ability to critically think. - Demonstrate strong attention to details and the ability to manage multiple tasks and priorities - Possess strong communication skills with the ability to engage patients effectively over the phone. - Must comply with all HIPAA rules and regulations - Other duties as assigned Qualifications - Certified Emergency Medical Technician (EMT) or Medical Assistant (MA) - Preferred 3-5 years of experience - Prior experience in an outpatient healthcare setting is strongly preferred - Experience with electronic medical records (EMR) systems - Proficient in basic technology platforms with the ability to navigate multiple systems The hourly range for this role is $20.00-$23.80 in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs).
