Role Description As a Wellness Coach, you will be responsible for providing personalized guidance and support to clients on their journey towards achieving optimal health. Your role will involve: - Conducting one-on-one coaching sessions with clients to assess their health needs and develop personalized wellness plans. - Providing guidance on healthy lifestyle choices, including nutrition, physical activity, stress management, and sleep hygiene. - Educating clients on chronic disease management, medication adherence, and preventive care. - Monitoring client progress and making necessary adjustments to wellness plans. - Collaborating with other members of the healthcare team to ensure coordinated care for clients. - Staying up-to-date with current health trends and research to provide the best possible support to clients. - Creating educational materials, workshops, and seminars to promote health and wellness within the community. Qualifications - Bachelor's degree in health education, nutrition, or related field. - Certified Wellness Coach or Health Coach credential preferred. - Minimum of 2 years of experience in health coaching or related field. - Knowledge of chronic diseases, healthy lifestyle promotion, and behavior change theories. - Excellent communication and interpersonal skills. - Ability to work independently and as part of a team. - Passion for helping others and making a positive impact on their lives. They will provide inbound and outbound telephonic support for care management team. Primary work will include outbound calls to patients to facilitate transitions of care, provide coordination services, fill gaps in care, and promote improvements to patient outcomes.
