About Flourish More than 10% of children and young adults will be diagnosed with serious mental health needs. The majority of these families struggle to get the care and support they need. Flourish (https://flourish.health/) is here to ensure that everyone has access to great care. About the role We are looking for a full-time Family Mental Health Guide to support our patients and their families. Our patients are children, teens and young adults living with serious mental illness, who are treated by a multi-disciplinary team of clinicians. You will be the front-line relationship with the primary family member, providing support, companionship, and coaching, as they care for their adolescent during our yearlong treatment program. Caregivers (family members) are too often overlooked in pediatric mental health care. We believe including them in our care model will bolster their ability to effectively support and care for their child, thus catalyzing change. You will be providing non-clinical, virtual support. We see Family Mental Health Guides as vital to the team’s success, and are building this role to be a sustainable, long-term career. What We Really Care About - Highly relational individual with strong interpersonal skills; demonstrated ability to build and maintain trusted relationships with people of varying backgrounds and lived experiences from a position of non-judgment & respect - Strong commitment to working from a position of humility, respecting the patient/families as the expert in their own lives - Adaptable to changing workflows as the clinic grows - Resourceful individual who can think quickly, navigate ambiguity, and learn while doing - Strong organizational skills and attention to a high level of detail - Self-awareness and demonstrated commitment to self-care; access to/willingness to access a community of support and/or resources that promotes longevity in the role - Willingness to give and receive feedback as part of cross-functional multidisciplinary team; we believe compassionate and direct feedback builds team trust and promotes a high quality of service - Comfort with technology; willingness to learn to use technical tools as part of the company’s internal and external communications and operations processes Some of the Responsibilities - Establish and maintain trusted relationships with Flourish Health patients/families utilizing a patient centered, strengths-based, and trauma-responsive approach - Apply experience using structured, manualized interventions to support caregivers of children with externalizing behaviors, offering consistent coaching and guidance. - Understand that as a growing clinic, there will be workflow changes and take changes in stride with adaptability, being able to prioritize and reprioritize when things change - Be a steadfast, caring, and non-judgmental presence, walking alongside patients/families as they live with a serious mental health condition while navigating life’s everyday challenges - Co-develop and support adherence to individualized care/engagement plans that support each patient’s self-identified needs and goals - Utilize app-based messaging and phone calls as primary means of engagement to meet patient and family member needs - Identify needs and provide mental health advocacy for patients in a variety of areas, such as school or work - Be an active participant in a multidisciplinary team of clinicians, doctors, and advocates supporting each patient and family; lead care coordination to include scheduling and communication for clinical appointments - Act as a conduit to a patients’s full care team; proactively identify needs for additional support and/or resources, and facilitate connection to the appropriate care team provider - Provide in-person coverage to support patient engagement with our virtual clinic when the assigned family guide is within a 2-hour radius and no other support or patient guides are available within the same radius. Education/Experience: - This position is open to individuals with Associate’s, Bachelor’s, or Master’s degrees - 5+ years of experience in a relevant field required, including 3+ years of experience working closely with caregivers of children with serious emotional disturbance and externalizing behaviors, providing guidance and practical support. - Bonus points (and we truly mean bonus, don’t let this deter you from applying) if you have demonstrated experience or interest working with: - Child or adults with serious mental illness - adolescents, young adults, and/or families - underserved communities with complex family situations - or if you have bilingual, Spanish and English speaking abilities - lived experience as a caregiver of children with behavioral health needs Schedule: Guides will have family-facing time as well as protected non-family facing time for collaboration with the care team, and other administrative tasks.
