• Design, build, and ship backend services, workflows, and integrations that power clinical charting, scheduling, task management, and care coordination • Own features end to end: from shaping the approach with product and clinical partners, through implementation, testing, rollout, and operation in production • Work primarily in a mature Ruby on Rails monolith, alongside supporting services and internal tooling • Build and maintain integrations with clinical and operational systems, including our EHR • Practice small-batch, continuous delivery: small PRs, short-lived branches, feature flags, and tight feedback loops that let us ship safely and frequently • Use agentic, AI-native development tools (e.g., Claude Code) as a core part of how you design, build, test, and review, without compromising engineering rigor • Write well-tested, observable, maintainable code, and instrument your work so we can understand how it behaves in production (we run on Datadog) • Raise the technical bar for the team through thoughtful code review, pairing, and mentorship of other engineers • Ensure the systems you build operate safely within HIPAA and broader healthcare regulatory requirements • Identify technical risks early and help drive mitigation before they impact care delivery

• Lead and scale clinical operations across physicians, APPs, dietitians, and clinical support teams, ensuring outstanding, evidence‑based care and a superlative patient experience. • Own clinical operating performance: SLAs, capacity, utilization, multi‑state workforce planning, and operational efficiency, in close partnership with Practice Operations and Finance. • Set clinical operations strategy and drive practice transformation (e.g., physician‑APP models, new service lines, workflow redesign) in collaboration with the COO, CMO, and Executive Leadership Team. • Provide strategic guidance to Product, Engineering, and Operations on tools and features that impact clinicians and patients, aligning the product roadmap with evolving care models.careers. • Lead, coach, and develop clinical leaders and managers, building scalable leadership infrastructure and promoting clinician wellness in partnership with the Director of Wellness. • Represent Form Health’s clinical organization externally (select conferences, client meetings, etc.) and support Sales and Marketing with clinical narratives and outcomes when needed. • While this role will be primarily focused on leadership duties, we would expect this person to maintain a very small longitudinal patient panel.

• Process biweekly/semi-monthly payroll for exempt and non-exempt employees, including clinicians, hourly staff, and corporate team members • Ensure accurate calculation of wages, overtime, bonuses, deductions, and benefits contributions • Maintain payroll system data integrity and manage updates for new hires, terminations, promotions, and compensation changes • Partner with TriNet and Finance on payroll reporting, reconciliations, and compliance support • Serve as the first point of contact for payroll inquiries, resolving issues promptly and with care • Administer all employee leave of absence programs, including FMLA, ADA accommodations, state and local leave laws, parental leave, personal leave, and workers' compensation leaves, ensuring compliance with federal, state, and company requirements • Partner closely with People Operations, Clinical Leadership, and Legal teams to coordinate pay impacts, benefits continuation, scheduling considerations, and reasonable accommodations while maintaining accurate leave records and ensuring a positive employee experience • Stay up to date on federal, state, and local payroll/tax laws, particularly in multi-state healthcare employment • Ensure compliance with healthcare industry-specific requirements (e.g., credentialing, licensure, and timekeeping standards).

• We are hiring experienced Nurse Practitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity. • Collaborate with physicians and registered dietitians to deliver exceptional team-based care.

• Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs). • You have your own panel of patients, and the role could also involve collaborating with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes. This role could involve APP collaboration and supervision. • Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions. • Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols. • Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs. • Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship. • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations. • Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine. • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care. • Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.
