Director, Care Delivery

• Lead and oversee daily operations of multidisciplinary care teams, including care coordinators, medical assistants, and nursing support staff. • Develop and implement operational strategies that improve patient access, care quality, and team efficiency. • Establish scalable processes and workflows to support organizational growth and evolving patient needs. • Accountable for operational performance through key performance indicators (KPIs) and service-level metrics. • Oversee provider utilization and capacity management across physicians and advanced practice providers for scheduling efficiency, panel management, and appointment access metrics. • Influence workforce planning and drive staffing frameworks to ensure optimal team coverage to meet patient demand. • Ensure seamless coordination between care teams and providers throughout the patient journey that enhance patient satisfaction and clinical outcomes. • Partner with cross-functional teams to implement new programs, workflows, and technology enhancements. • Drive continuous improvement initiatives using data and performance insights in scaling and evolving Form Health's virtual care model. • Foster a culture of accountability, collaboration, continuous improvement, and patient-centered care. • Support change management efforts and organizational initiatives that improve care delivery and operational effectiveness.

Job Requirements

  • Bachelor's degree required; Master's degree in Healthcare Administration, Business Administration, or related field preferred.
  • 7+ years of progressive leadership experience in healthcare operations, care delivery, or clinical operations.
  • 3+ years of experience leading virtual care, telehealth, or digital health operations preferred.
  • Experience managing multidisciplinary healthcare teams, including care coordinators, medical assistants, nursing teams, and provider operations.
  • Demonstrated success in provider capacity planning, utilization management, and operational performance improvement.
  • Strong analytical and data-driven decision-making skills.
  • Proven ability to lead teams through growth, change, and process transformation.
  • Exceptional communication, collaboration, and leadership skills.
