Role Description We are looking for full-time Nurse Practitioners with prior experience managing patients with heart failure. You will manage a cohort of patients with a diagnosis of heart failure and leverage our technology and first in class medication optimization algorithms to help patients with heart failure and other chronic conditions (such as hypertension and diabetes) live a healthier life and reduce hospital admissions. We are seeking full-time coverage (40 hours per week) Monday through Friday (8:30 am - 5:00 pm or 9:00 am - 5:30 pm), within the employee’s local time zone. What you’ll be doing: - Virtually evaluate patients entering the program and assist in creating a care plan based on patient specific needs and predesignated protocols. - Prescribe and titrate medications based on evidence-based guidelines and company approved standards / regulations. - Optimize guideline-recommended medical therapy for patients in all stages of heart failure. - Respond to alerts escalated from the nursing team related to patients’ deviations in vitals and physiologic data and symptoms. - Independently manage changes in clinical status that are within the APP’s scope of practice while escalating out of scope clinical changes to a physician. - Liaise with the nursing team to ensure proper medications, labs and virtual appointments are ordered and communicated to the patient. - Provide education to patients about healthy behaviors and self-management. - Providers will be responsible for creating meaningful and measurable improvement in disease control rates (including hospitalization reduction). Qualifications - 3+ years as a Board-Certified Adult or Family Nurse Practitioner, with prior experience in adult chronic care management, specifically in cardiology and heart failure. - Completed an accredited NP program with a national certification. - Active licensure in at least one of the following states is required: NC, TN, IN, OH. - Active licensure in at least 2-3 of the following states is strongly preferred: GA, KY, PA, VA, TX, FL, and CA. - Willingness to obtain additional state licenses as needed. - Prior experience with management of heart failure patients, including prescribing and managing evidence-based therapies, is required. - Previous telehealth experience strongly preferred. - An attitude and self-motivated work ethic that meets the core values and goals of the company. - Eager to work in a fast-paced, quickly evolving environment with a high tolerance for change and willingness to adapt. - Ability to work from home in a HIPAA compliant environment is required. - Ability to learn and proficiently use multiple EMRs and various communication applications, including Google products.
