Role Description We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. In this role, you will: - Lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure. - Ensure adherence to evidence-based guidelines for type 2 diabetes and hypertension. - Oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider. - Optimize treatment plans, adjust medications, and determine the appropriate level of care. - Deliver timely, patient-centered, and scalable remote care. Qualifications - 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center. - Experience managing CHF with GDMT. - Active ANCC or AANP certification. - Master’s Degree as a Nurse Practitioner. - Home office setup with consistently stable Wi-Fi with strong upload and download speeds. - Ability to thrive in an environment founded on trust, autonomy, and direct communication & feedback. - Experience working in a CHF bridge clinic or T2D clinic, a startup environment, and working with remote patient monitoring technology is preferred. - Comfortable using AI-assisted clinical tools while maintaining clinical judgment as the primary guide for every patient decision.
