Home Health Telehealth Registered Nurse – Part-Time

Graham Healthcare Group

• Provides overall triage assessment, case management and coordination via telephonic and telehealth care to patients. • Assures optimal coordination of patient care across the continuum for all disease management and triage patients as assigned. • Provides telephonic triage to all incoming patient/caregiver calls as assigned. • Works in conjunction with the Clinical Supervisors, Customer Service staff, field staff, and on-call staff to coordinate care to best meet the patients’ needs efficiently. • Acts as a patient advocate in coordinating care within and outside the organization. • Communicates with all providers involved in patient care of the Disease Management/Triage patients to assure coordination of care.

Job Requirements

  • Licensed RN with current state license.
  • Excellent telephone and communication skills required.
  • 2-3 years of clinical practice in their respective field.
  • 1-3 years of home health/hospice experience.
  • Knowledge of disease management and chronic disease processes.
  • Prior management experience preferred.
  • Ability to work independently and within a multidisciplinary team.
  • Must have and maintain in good standing professional license, certificate, or registration, as applicable.
