Compensation The Wage Range for the role is $22.50 - $31.00 per hour. If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position. New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year. New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour. Washington Exempt: $80,168.40/year. Centralized Home On Call Coordinator

Role Description - Using advanced knowledge and problem-solving skills, ask the appropriate probing questions to determine, diagnose and solve patient and team support issues. - View all Outlook for urgent communications related to missed treatment, hospitalization, and infection for adherence to clinical P&P and appropriate resolution. - Escalate any concerns to Manager for clinical judgment appropriateness adherence to patient safety standards. - Monitor Outlook for local field communications and address all emails in relation to distribution list; escalate clinical concerns. - Schedule management - complete and update the Apello schedule daily and weekends. - Daily report email - prepare and distribute the daily report email including: - Key operational updates – mutual communication - Pertinent patient safety issues - Charge nurse assignments - Team break schedule - Morning clinical support - Be available daily between 8:00 AM and 11:00 AM Eastern Standard Time for clinical call concerns and immediate support needs in addition to the scheduled-on call nurse as needed. - When necessary, escalate Team or Patient concerns to Manager. - Team communication and support: - Hosts biweekly office hours to address technical workflow concerns, process improvements, and provide learning opportunities for the team. - Directory maintenance: - Continual maintenance and updates - Palmer directory worksheets - Distribution list support - Training and onboarding: - Assist with training of new team members - Scheduling onboarding and shadowing experiences - Ensuring understanding of workflow - Ensuring adherence and understanding of clinical P&P - Supporting acclimating new teammate to a full shift workload - Responsibilities may expand or evolve over time to meet the needs of the team.
