Responsible for assisting individuals and families through the single point of entry process, which includes initial intake, eligibility determination, assessment, options counseling and referral. Administers initial functional assessment, utilizes the Eligibility Determination Instrument (EDI) to clarify eligibility issues and provides back up support for annual functional assessments. Responds to requests for information and educates applicants and stakeholders about the I/DD system. Engages and facilitates informed decision-making about accessing long-term services and supports in the community with individuals/guardians. Service Access - Assists individuals in accessing the I/DD service system - Schedules appointments, assessments, and follow-up notifications - Collects necessary information for eligibility determinations - Determines the need for additional eligibility information on a case by case basis - Documents information related to contacts and transactions - Processes and communicates eligibility determinations in a timely manner - Maintains a working knowledge of the I/DD eligibility process and associated criteria - Participates in all statewide eligibility meetings - Coordinates case transfers into Sedgwick County Education & Referral - Responds to requests for information - Educates stakeholders on the available service options and the status of funding - Educates stakeholders on local and state-wide community resources - Assembles and distributes resource packets containing affiliate service provider information - Processes targeted case management referrals based on consumer/guardian selection - Provides documented options counseling for new access, referrals and provider changes Assessment - Completes the Kansas Department for Aging and Disability Services (KDADS) required training prior to performing assessments - Coordinates and administers initial functional assessments - Coordinates and administers functional assessments for individuals being offered access to HCBS-IDD Program funding - Coordinates and administers request for functional assessment from other CDDOs for individuals residing in Sedgwick County - Administers and scores Eligibility Determination Instrument assessments - Participates in all Statewide functional assessment roundtable meetings/trainings Minimum Qualifications: Bachelor’s degree from an accredited college or university in Social Work or a related field. At least two years of experience as a case manager or any equivalent combination of training and experience that provides the required knowledge, skills and abilities. Education may be substituted with work experience in the Social Services field at a rate of 1,040 hours for each semester of college. The Kansas Department for Aging & Disability Services (KDADS) may grant an exception to the above minimum qualifications. Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance. We’re looking for passionate, dedicated telephone counsellors who will share our values of trauma-informed, child-centred domestic and family violence practice. In these part-time max-term 6 month roles, with strong potential for extension, based remotely working from home, you’ll work as part of the 24/7 national 1800RESPECT telephone counselling service, providing specialised trauma counselling services to those whose lives have been impacted by domestic and family, sexual assault, and/or workplace sexual harassment. Role Description The Fraud Triage Specialist position plays an essential role in ensuring the overall profitability of the insurance, benefit, and warranty programs within the Connected Living Organization.
