Role Description As a Resource Pediatric Registered Nurse at Imagine Pediatrics, you serve as a key clinical partner within the Patient Engagement (Outreach & Enrollment) team, supporting the re-engagement of previously enrolled patients and families. This role focuses on providing real-time clinical support to patients who are newly reconnected to services, ensuring immediate access to care, guidance, and triage at the point of engagement. In this role, you will collaborate closely with Patient Engagement Agents, Community Health Workers, and interdisciplinary care team members, operating within a non-traditional structure aligned to patient engagement and operational priorities. You will act as a primary point of contact during re-engagement, delivering timely, family-centered clinical support through virtual touchpoints and ensuring patients are quickly connected to appropriate care pathways. - Provide real-time clinical support and triage for newly re-engaged patients, ensuring immediate response to clinical needs following outreach and enrollment interactions. - Deliver proactive, professional, and compassionate care, serving as an accessible first point of contact for patients and caregivers. - Apply a family-centered care approach, meeting patients where they are and supporting individualized care needs. - Demonstrate cultural competence and sensitivity, effectively supporting diverse populations and leveraging appropriate resources as needed. - Build trusting relationships with patients and families to support engagement, understand needs, and encourage ongoing participation in services. - Perform a comprehensive assessment of patients’ clinical, psychosocial, discharge planning, and financial needs, establishing appropriate clinical milestones and goals. - Collaborate with physicians, APPs, and interdisciplinary team members to develop and execute care plans aligned to patient needs and clinical goals. - Coordinate with Resource specialists, Behavioral Health, and other interdisciplinary partners to address complex needs and ensure appropriate referrals and follow-up. - Monitor and evaluate the effectiveness of care plans, gathering input from relevant sources to ensure care is delivered in an accurate, safe, timely, and cost-effective manner. - Document all assessments, interventions, and patient interactions accurately and in a timely manner. - Meet established monthly re-engagement productivity and engagement goals, including completed patient encounters, effective warm handoffs, and sustained caregiver engagement. - Contribute to longitudinal engagement outcomes by supporting a target percentage of re-engaged patients to complete a clinical visit within three months of initial re-engagement. Qualifications - Licensed RN in at least one state with eligibility to register for other state licensures. - Graduate with a bachelor’s in nursing from an accredited university preferred. - Minimum 1 year care coordination or case management required. - Familiarity with Medicaid regulations and services required. - Experience in Value Based Care highly preferred. - Virtual care experience a plus. - Bilingual Spanish is required. • Conduct focused and thorough assessments of patients with general cardiology and heart failure needs through virtual consultations including ordering diagnostics as needed, interpreting labs and imaging data, and developing a treatment plan in collaboration with the specialty care clinic team. • Formulate accurate diagnoses and develop individualized treatment plans for patients with heart failure and general cardiology clinical conditions, including medication management, volume monitoring and management, and lifestyle modifications. • Initiate, titrate, and manage guideline-directed medical therapies (GDMT) for a diverse group of members with heart failure. • Provide proactive management of heart failure and comorbid conditions, and address potential barriers to optimal care to mitigate the risks of heart failure exacerbation and hospitalizations. • Assess and intervene in weight fluctuations and acute volume overload management of patients who are enrolled in the clinic. • Assess, diagnose, and manage the range of general cardiology conditions that are suitable for virtual care including stable coronary artery disease, hypertension, hyperlipidemia, stable atrial arrhythmias, peripheral arterial disease and chronic venous disease. • Collaborate closely with other members of the care team including PCPs, cardiologists and other Devoted team members including pharmacy, clinical nursing, social work, as well as interfacing with family members and caregivers to coordinate holistic care for the member, to ensure continuity of care and deliver a collaborative care plan. • Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines. • Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in cardiovascular care.
