Hospice RN Case Manager
Rockstar
• Completing initial and ongoing assessments to determine hospice needs • Creating, updating, and implementing individualized care plans • Delivering skilled nursing care, medications, and symptom management • Supporting and educating patients and caregivers on comfort-focused care • Serving as case manager for an assigned caseload • Coordinating with physicians, families, and the interdisciplinary hospice team • Participating in on-call rotation and team meetings • Supervising and supporting hospice aides
Job Requirements
- Graduate of an accredited nursing program; current RN license in Missouri (or eligible)
- 1–2 years of recent acute care experience (home health/hospice preferred)
- CPR certification
- Valid driver’s license and insured, reliable vehicle
- Excellent communication, assessment, and problem-solving skills
- Compassionate approach to end-of-life care
