Primary Responsibilities: - Make outbound calls and receive inbound calls to assess members' current health status and gaps in care - Completes assessments evaluating members for clinical and social needs timely and effectively - Follows model of care requirements as outlined in policies, procedures, and job aids - Evaluates members for ongoing needs and completes referrals as appropriate - Manages, communicates, and updates the plan of care with members - Collaborates with other programs and departments as necessary - Collaborates with Primary Care Physicians regarding medication concerns, questions, or other member needs - Ability to work independently and follow program processes You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: - Current, unrestricted RN compact license - 3+ years of experience in a hospital, acute care, or direct care setting - Basic level of experience with Microsoft Office (Word, Excel, Outlook) - Designated workspace and access to install high-speed internet via hardwire in home Preferred Qualifications: - BSN - Certified Case Manager - Case Management experience - Experience or exposure to discharge planning, utilization review, concurrent review or risk management - Experience in a telephonic role - A background in managed care *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
