The Medication Adherence Technician applies knowledge and skills to engage in in-depth discussions with patients about medication adherence to identify and resolve patient adherence barriers. The incumbent in this role works with providers, clinical pharmacists and the pharmacy department to provide a coordinated adherence packaged product for patients requiring additional support establishing and maintaining compliance to their medication regimens. He/She is accountable for providing an unsurpassed patient experience by communicating to patients/providers on program specifics and arranging for prescription processing within the pharmacy; with the ultimate goal for patients to obtain an organized product they will understand allowing them to reduce or eliminate any adherence challenges. The Medication Adherence Technician focuses on providing consistent visibility and a presence to the target clients and patients.ESSENTIAL JOB DUTIES/RESPONSIBILITIES: - Makes outbound calls to patients and healthcare professionals regarding patient medications, adherence, and barrier removal. - Works diligently to remove barriers to ensure prescription regimens are provided to patients. This may involve analysis, research, and problem-solving to obtain resolution. - Interacts with healthcare professionals of all levels involving patient medications. Calls the pharmacy or provider to assist patient with medication refills, medication synchronization, or 90-day supply requests. - Captures and supports medication adherence patient prescription requests/needs. - Facilitates and/or coordinates the process whereby prescriptions are sent into the pharmacy pharmacy for patients, processed appropriately, adjudicated to third party insurance, error resolution completed, packaged and prepared for dispensing to the patient. - Follows established medication adherence workflows, including but not limited to: following up with patients requesting enrollment and patients currently enrolled to initiate or continue delivery of medication regimens. - Ensures assigned prescription processing tasks are accurate and reviewed by supervising personnel as required before dispensing to patients. - Motivated to meet set targets as established by leadership. Completes minimum daily goals for outreach (telephonic, email, mail) to patients and clinicians for Medication Adherence measures. - Provides feedback and/or solutions on how to improve the medication adherence patient/client experience. - Assists with training programs for clinical personnel involved in Part D HEDIS measures in the centers of ChenMed/JenCare/Dedicated. - Performs other duties as assigned and modified at manager’s discretion. KNOWLEDGE, SKILLS AND ABILITIES: - Foundational knowledge and understanding of standard medication adherence practices, protocols, techniques, functions and processes - Excellent interpersonal skills, telephone communications skills and pharmaceutical calculation skills - Excellent written, verbal, listening and interpersonal communication skills - Good organizing and prioritizing skills - Strong time-management skills and keen eye to detail - Must possess a customer service mindset with the ability to remain patient, calm and empathetic is challenging situations - Ability to actively listen and understand patient questions and provide accurate answers or solutions. - Ability to make sound decisions and conduct follow ups for problematic situations that may arise during the established process - Ability to practice discretion and care on a regular basis when working with sensitive/confidential data pertaining to the patient, department, or company - Ability to interact with all levels of internal and external parties, to convey information correctly and always represent ChenMed and its entities in the most favorable manner - Capable of multi-tasking in a fast-paced, ever-changing healthcare environment - Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software - Ability and willingness to travel locally, regionally, divisionally or domestically up to 10% of the time - Spoken and written fluency in English EDUCATION AND EXPERIENCE CRITERIA: - High School Diploma or equivalent required OR additional experience above the minimum may be considered in lieu of the required education on a year-for-year basis - Must meet at least one of the following requirements: - Certification as a Pharmacy Technician or Medication Assistant with a minimum 6 months of related work experience OR - A minimum of 2 years of work experience in a medication dispensing/pharmacy setting or call center PAY RANGE: $14.3 - $20.42 HourlyThe posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.
