Job Description

Justice Program Coordinator

Mindoula

• Efficiently receive, review, and process incoming referrals across all 37 WA Justice Contract collaborations, verifying eligibility and ensuring swift entry into the program • Attend and actively participate in warm hand-off case management meetings with justice partners, care teams, and community providers to ensure smooth care transitions for members entering behavioral health services • Serve as the primary scheduler for member intake assessments and ongoing follow-up appointments, managing complex calendars and reducing no-show rates through direct engagement • Collaborate directly with Program Directors, Care Extenders, internal Population Health Coordinators, and external health plan/justice partners to streamline workflows and maintain clear lines of communication • Handle high-volume communications via phone and email, serving as a primary liaison between justice-involved members, care teams, and external stakeholders with high empathy and professionalism • Maintain accurate referral logs, tracking enrollment metrics, appointment completions, and care coordination efforts within Electronic Health Record (EHR) systems and tracking sheets • Maintain and update a centralized database of community resources specifically tailored to justice-impacted populations (e.g., re-entry services, behavioral health, housing support) • Provide cross-coverage for other SPHM Program Coordinators during peak volumes or absences as needed.

Job Requirements

  • Bachelor’s Degree required (Degree in Human Services, Social Work, Criminal Justice, Healthcare Administration, or related field preferred)
  • Proficiency in Google Workspace, Microsoft Excel/Sheets, and data analytics or referral tracking tools
  • Prior experience navigating Electronic Health Record (EHR) systems within a strict, HIPAA-compliant healthcare environment
  • Strong background, knowledge, or deep interest in justice-involved populations, re-entry initiatives, and behavioral health/substance use support
  • Superior organizational and multi-tasking skills, with proven experience managing high-volume scheduling across complex, multi-site programs
  • Exceptional verbal and written communication skills with a strong foundation in empathetic, trauma-informed customer service
  • Ability to thrive in a fast-paced, high-urgency remote environment while maintaining accuracy and accountability.

Role Description The Enterprise Release Analyst is responsible for the implementation and maintenance of system change control processes, collaborates with Business Requesters, Workday Teams (Finance, HCM and Integrations) and relevant stakeholders from Global Controllership and SOX Team, and ensures the compliance with SOX requirements and internal policies throughout the change management lifecycle. Responsibilities - Collect change requests submitted by Business Requesters from different regions (Americas, EMEA and APAC), and ensure every change request has appropriate approvals and supporting documents. - Understand Business Requesters’ exact requirements, assign change requests to Workday Team Members (Finance, HCM and Integrations) and tag change requests to monthly releases or off-cycle releases or release exceptions. - Follow up with Workday Team Members (Finance, HCM and Integrations) to collect UAT signoffs and supporting documents for change requests. - Coordinate Release Approvers to review and approve changes requests included in monthly releases, off-cycle releases, and release exceptions. - Create Change Tickets in ServiceNow for monthly releases, off-cycle releases and release exceptions, and attach Business and IT approvals and supporting documents to the tickets. - Attend weekly Change Advisory Board (CAB) meetings, present the Change Tickets and supporting documents to CAB for their review and approval, and clarify CAB’s questions (if any) on individual tickets. - Post CAB approvals, coordinate Workday Team Members (Finance, HCM and Integrations) to implement changes in Production in terms of the release schedule. - For any issues and/or additional change requests raised by Business Requesters during the hypercare period to initial change requests, collaborate with Workday Team Members (Finance, HCM and Integrations) to perform additional testing and obtain additional approvals by Business and IT Approvers. - Create and distribute release schedules and release notes to relevant recipients. - Participate in regular team meetings and ad hoc meetings to discuss change requests and confirm the Go(s) or No-Go(s) with relevant stakeholders. - Take care of ad hoc task assignments when needed.
