Role Description This job leads and supervises the documentation excellence program for designated service lines comprised of multiple functional areas including hospital and professional coding, hospital and professional CDI, and documentation quality to meet system goals. This job is responsible for overseeing initiatives to improve processes, maximize efficiency, serve internal and external customers and maintain high employee engagement. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Qualifications - Required – High school diploma or equivalent - Preferred - Additional degrees, business training experience and/or certifications may be combined to meet minimum qualifications Requirements - Required – Three years of experience in healthcare management or related field - Preferred - One to two years of experience in revenue cycle processes - Preferred – Current RN License in the state of practice; Current Registered Health Information Technician (RHIT); Registered Health Information Administrator (RHIA); current CCS or CPC certification Knowledge Skills and Abilities (KSAs) - Must have computer skills and dexterity required for data entry and retrieval of required job information. - Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout required job processes. - Must be proficient with Windows-style applications, keyboard, and various software packages specific to role. - Strong interpersonal skills. - Must be able to work independently, set priorities and maintain complete confidentiality. Job Duties - Supervises the daily operations of the department. - Supervises and leads team by monitoring individual performance, coaching employees, establishing and enforcing policies and procedures, resolving employee issues and developing team members. - Implements and monitors quality control measures. - Monitors and documents performance metrics. - Works collaboratively with physicians, administration, nurses and support staff to improve documentation. - Other related duties as required. Physical and Environmental Demands - Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. - Normal routine involves no exposure to blood, body fluid or tissue and as part of their employment, incumbents are not called upon to perform or assist in emergency care or first aid. - The incumbent has no occupational risk for exposure to communicable diseases. - There may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. Primary Responsibilities: - Reviews encounters in the electronic medical record to ensure accuracy and appropriateness of clinician documentation using relevant risk adjustment program standards. - Utilizes abstract reports to substantiate HCC diagnoses that have been billed and/or are considered relevant but do not appear on the patient’s problem list. - Provides concurrent and retrospective abstraction for all Atrius Risk Adjustment programs. - Independently conducts chart reviews for supporting documentation of diagnosis codes which can be added to Problem List.
