Job Description

Care Partner

Within Health

• Support with Admissions and call handling as needed • Coordinate care and program services with patient, family, and treatment team • Organize, plan, and provide structure around client-specific experiential opportunities • Meet with client and/or families for virtual sessions and check-ins • Provide families with support and motivation • Model appropriate table behaviors to clients struggling with eating disorders • Provide families with one-on-one coaching in how to manage their loved one’s behaviors • Provide in-the-moment feedback by utilizing between-session messaging to motivate and support clients and families • Alert medical and clinical professionals to intervene during emergencies • Maintain strict client confidentiality • Participate in Treatment Team meetings • Attend or lead groups as appropriate • Attend supervision and department meetings • Participate in initial and ongoing training • Provide case management for members, ensuring effective communication with those involved in the recovery process, including school administrators, law enforcement, attorneys, etc. • Document all client interactions appropriately and within designated timeframes • Other related duties as assigned based on need and availability

Job Requirements

  • Associate's degree in behavioral health or related field, or 2 years experience
  • Ability to treat patients with dignity and empathy
  • Strong listening and communication skills
  • Acute observation skills
  • Computer literacy
  • Willingness to treat clients with Eating Disorders and Disordered Eating
  • Ability to demonstrate understanding of a variety of models and theories of eating disorders, trauma, mental illness, and related issues
  • Understanding of diverse cultures and gender specific issues and ability to incorporate needs of gender and culturally diverse groups into practice setting
  • Excellent organizational and time management skills
  • Ability to prioritize workload and work independently
  • Must have reliable internet connection
  • Must be comfortable operating a computer and smart-phone and navigate applications within macOS and iOS
  • Must be comfortable communicating with colleagues via chat, telephone, and video calls
  • Must be able to sit for the majority of the shift

Role Description We are seeking a dynamic, collaborative and innovative Change Management Practitioner to support the IAO transformation. This role will be integral in fostering engagement, aligning stakeholders, and ensuring sustained change. The IAO transformation requires changes across people, process and technology, with much of the technology change centered around the overhaul of our ERP and Supply Chain Planning tools that will enable our transformation to a more independent fabless company and systems foundry. As a Change Management Practitioner, you will drive change management activities, collaborating closely with program teams, project managers, and most importantly, our end users, managers and leaders. You will be responsible for, but not limited to: - Apply change management principles and standards and deliverables to meet diverse stakeholder needs and mitigate potential resistance. - Assess stakeholder readiness for change and proactively execute change management interventions that increase adoption readiness and mitigate resistance. - Develop a compelling business change narrative that resonates at all stakeholder levels. - Execute the change support network, including Change Agent Network, key stakeholder coalitions and front-line manager enablement across Intel, in partnership with fellow change management practitioners. - Consult and uplevel program training strategies to the IAO driven business model, manage development process and material deployment to drive user knowledge and ability to perform in our new business strategy. - Consult on communication strategies coordinate and execute IAO uplevel communication plans and materials for specific initiatives to foster stakeholder engagement, increase adoption, reinforce future state ways of working and minimize business disruptions.
