Pre-Visit Planning Assistant

Role Description The Pre-Visit Planner oversees and coordinates preventative care services and community resources for patients and their families to achieve good health. Within CentraCare Clinics, the goal of the pre-visit planner is to help patients achieve optimal health by planning patients care using evidenced based guidelines for care, (i.e. Hgb A1C or immunizations as appropriate) or gathering information pertinent to the patients care from other sources. - Schedule: Full-time | 80 hours every two weeks | Remote - Day shift | 7:30am-4pm - Fully remote after a training period on site Qualifications - High school diploma or equivalent required - 3 years of experience in a medical setting with knowledge of medical terminology, medical records and office skills including phone etiquette and computer keyboarding Benefits - Starting pay begins at $18.40 per hour; exact wage determined by years of related experience - Pay range: $18.40 - $27.61 per hour - Tuition reimbursement and college grant programs available - Full-time benefits: Medical, dental, PTO, retirement, employee discounts and more!
