About the job Internal Title: Lead Care Manager We are looking for a highly skilled Lead Care Manager (LCM) to join our team in the San Francisco Bay Area, CA. Reporting to the Director of Enhanced Care Management, ECM Clinical Manager, and/or ECM Program Manager, the LCM acts as the client’s main care coordinator, collaborating closely with providers including physicians, specialists, pharmacists, and social service agencies to ensure aligned, comprehensive support tailored to the client’s needs. The LCM manages client cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner. The LCM collaborates and communicates with client’s caregivers/family support persons, other providers and others in the Care Team in order to promote wellness, recovery, independence, resilience, and member empowerment, while ensuring access to appropriate services and maximizing member benefit. This is a hybrid position that requires traveling throughout the San Francisco Bay Area, including San Francisco, Marin, and San Mateo Counties. This is a critical role and we're looking to fill it as soon as possible.
