Full- Time Clinical Supervisor: Nursing Anticipated Coverage area: Sacramento In this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of esculation ensuring support is easily accessible in a timely fashion; You will participate in the Identifation, development, design, and implementation of nursing educational programs to support clinical excellence and determine success measures and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction. You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering inisghts to improve patient outcomes and reduce readmissions. This is a hybrid of field work and on site based at 21HHS office: Office: Spend 2 days per week in the office for administrative and team management duties. Fieldwork: Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers What you'll do as the Clinical Supervisor - Nursing - Responsible for in the field oversight of clinicians who provide care and treatment to patients in their place of residence. - Daily patient activities: Handles incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs. - Coordinates training and educational programs for nurses within the field to improve clinical outcomes and competencies. - Assesses staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff. - Functions as the first point of contact for field clinicians to assist in patient care escalations etc. - Coordinates at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes. - Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. - Reviews patient’s clinical diagnosis, medications, procedures and clinical course. - Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
