RN – Aging Life Care Manager
Aging Well Eldercare
• Conduct client assessments and develop care plans • Visit clients and support ongoing care needs • Communicate regularly with families and providers • Provide guidance during times of transition or change • Provide occasional weekend on-call support
Job Requirements
- RN license (MD required, DC a plus)
- CCM or CMC certification a plus
- Ability to travel for in-person client visits in the D.C. Metro area
- Experience in geriatrics, home health, hospice, or related fields
- Highly organized and comfortable managing multiple moving parts
- Comfortable using technology (e.g., Google Workspace, care management systems)
- Builds strong relationships and communicates thoughtfully
- Demonstrates emotional intelligence and healthy boundaries
