RN – Aging Life Care Manager

Aging Well Eldercare

• Conduct client assessments and develop care plans • Visit clients and support ongoing care needs • Communicate regularly with families and providers • Provide guidance during times of transition or change • Provide occasional weekend on-call support

Job Requirements

  • RN license (MD required, DC a plus)
  • CCM or CMC certification a plus
  • Ability to travel for in-person client visits in the D.C. Metro area
  • Experience in geriatrics, home health, hospice, or related fields
  • Highly organized and comfortable managing multiple moving parts
  • Comfortable using technology (e.g., Google Workspace, care management systems)
  • Builds strong relationships and communicates thoughtfully
  • Demonstrates emotional intelligence and healthy boundaries
