Description Description Basic Function: Therapists will provide mental health/behavioral health services to children and families, including individual and family therapy, and consult with agency staff as appropriate. Therapist will provide both community and/or telehealth services. We are looking for therapists across the state. Licensed therapists may perform the above services using a secure tele-health video platform from their home or office location. In-person office sessions may be required for clients within the local area when virtual sessions may not be appropriate. Principal Responsibilities: Therapists are professional experts in mental health who are fully licensed to screen, diagnose and treat mental health conditions. They support members by providing evidence-based psychotherapy. Core Responsibilities and Duties: - Manage a panel of clients providing evidence-based assessment, treatment plans and therapeutic interventions, meeting or exceeding completed consult expectations as outlined in with Medicaid Standards and Growing Home Southeast policy and procedures. - Develop and update clinical treatment plans in collaboration with members. - Diligently track your clients’ progress and serve as an accountability partner. - Engage in discharge planning upon conclusion of therapy program by providing case management services, external referrals, and coordination with other providers. - Respond to crisis situations by following established protocols and developing safety plans with clients. Additionally, continually assessing program eligibility and providing referrals, when needed, for follow-up intervention services. - Effectively communicate with team members to address client concerns, limitations, treatment progress and other concerns. Report any concerns regarding clinical performance and inability to meet expectations to Clinical Director immediately. - Collaborate with other team members to apply an integrated care model to client treatment plans to enhance progress and treatment outcomes. - Provides Individual and Family therapy to children and families referred from the community or internally. - Submit documentation of all services in accordance with regulations determined by Growing Home Southeast, South Carolina Department of Health and Human Services, and other regulatory authorities. - Provides clinical consultation with agency staff as needed and appropriate. - Communicates with Clinical Director concerning availability to accept new referrals. - Follows all ethical standards and agency procedures for abuse reporting and crisis situations. - Other duties as assigned. Education, Knowledge and Skills: Must be provisionally or independently licensed in South Carolina. Preferred minimum of two years of experience working with children and families of various backgrounds. Excellent written and oral communication skills, computer proficiency, and must maintain documentation according to SCDHHS standards. Trained in trauma-informed care or experience working with clients with a history of trauma preferred. The ability to serve younger clients with play therapy experience a plus. The Therapist must be committed to understanding and conducting work consistent with Growing Hope's mission, values and core principles, and exhibit organizational citizenship. They must also have the ability to forge mutually respectful partnerships; conviction about the capacity of people to grow and change; the ability to maintain the helping role of the practitioner to meet the needs of the persons served or other family members; and the ability to work in partnership with other team members. We're currently looking for an independently licensed mental health professional to join the Care Navigation team (LCSW, LMFT, LPC required) to support adult and adolescent members seeking support with posttraumatic stress disorder (PTSD) and complex trauma. Reporting to the clinical lead of the Trauma Specialty Care Program, the Trauma Specialty Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health’s innovative Trauma program. The role may also support teens 13 to 17 and their caregivers. This is a full time position that is fully remote. The primary shift for this position is Monday-Friday, 9-5pm PT. What you’ll do: - Developmentally appropriate telephonic and telehealth screening, brief intervention, and referral of members to a trauma specialty treatment provider. - Apply active listening, psychoeducation, and motivational interviewing skills to understand each member’s or family’s needs and provide support as they set goals. - Utilize Judith Herman’s phase based trauma treatment model to provide stabilization support to members in their process of engaging in care. - Employ motivational interviewing to assist members in engaging in care. - Identify an appropriate level of care, facilitate referral, and coordinate care with Spring Health providers and, when needed, community resources and schools. - Provide consultation regarding mental health concerns and family/relationship issues, as appropriate to member needs. - Balance time with the adolescent and caregivers (individual and joint conversations) to support the family system while honoring the teen’s voice. - As necessary, provide in-the-moment support and developmentally appropriate safety planning for members with safety, risks or crisis. - Utilize written communication skills to interact with members, caregivers, and providers through email. - Document information, maintain accurate records, perform follow-ups, and track progress within several systems – including an EHR – within required timeframes. - Identify and problem-solve issues that serve as a barrier or disruption to care. - Interact with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a smooth member experience. - Assisting other specialty care teams (SUD, Mood and Anxiety, and Eating Disorders), as needed. - Participate in staff meetings, case consultations, and trainings. - Assist with onboarding and mentoring new team members. - Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards. What success looks like: - Maintain high member satisfaction, as measured by survey responses. - Retain members throughout their course of care by maintaining consistent outreach and contact, as appropriate. - Triage and enroll members into appropriate LOC, based on presenting problems and goals. - Outreach adult members via phone and email to schedule triages. What you’ll bring: - Independent clinical mental health license firmly required (LCSW, LMFT, LPC, or equivalent). - Minimum of 2+ years of clinical care experience working with PTSD and Complex Trauma in adult and adolescent populations in outpatient or intensive outpatient settings. - Experience with motivational interviewing, diagnostic assessment, and triage. - Knowledge of phased-based trauma treatment models and EBPs for PTSD (e.g. Prolonged Exposure, Cognitive Processing Therapy, EMDR, ART, etc). - Experience working with parents and caregivers, including providing psychoeducation, motivational work, and collaborating with families to support the adolescent’s treatment and overall wellbeing when the teen is the identified patient. - Excellent listening, written and verbal communication skills. - Acknowledge and embrace diversity and inclusion. - Experience working with diverse populations. - Motivated to go above and beyond for members to provide a high-touch experience. - Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks. - Able to multi-task and context switch!
