Role Description Buscamos un Care Coordinator responsable de onboardear a cuidadores y pacientes, mantener comunicación continua por chat interno o llamadas grabadas, evaluar necesidades, priorizar inquietudes y coordinar soluciones con equipos clínicos y recursos comunitarios. El objetivo es reducir la carga del cuidador comprendiendo sus necesidades y asegurando que la plataforma las cubra eficazmente. - Registrar y guiar a cuidadores y pacientes en la plataforma. - Mantener perfiles actualizados. - Comprender las necesidades de personas con demencia y cuidadores, brindando apoyo empático y centrado en el paciente. - Actuar como punto de contacto principal y abogar por resoluciones oportunas. - Conectar a pacientes con recursos comunitarios y facilitar referencias clínicas. - Brindar apoyo en la instalación y uso de dispositivos en el hogar. - Analizar datos del sistema en casa y activar protocolos clínicos establecidos. - Registrar toda la información exclusivamente en la plataforma, garantizando cumplimiento de HIPAA. - Reportar incidencias, apoyar mejoras de procesos y contribuir a la validación del sistema. Qualifications - Formación en Enfermería o Trabajo Social. - Experiencia previa como cuidador/a, preferiblemente con personas con demencia. - Experiencia sirviendo a población en Estados Unidos y conocimiento del sistema de salud y seguros estadounidenses. - 3 años de experiencia trabajando con pacientes en clínicas, servicios al cliente o con equipos clínicos. - Inglés avanzado. - Excelentes habilidades de organización, comunicación y manejo del tiempo. - Capacidad para usar una plataforma basada en IA y sistemas SAS a nivel básico. - Manejo de herramientas de investigación y recopilación de información (ChatGPT, Gemini). - Conocimiento y cumplimiento estricto de HIPAA. - Competencia digital básica y habilidad para documentar con precisión. - Conocimiento de terminología médica.
