Role Description As an Outreach Specialist, you are responsible for locating and connecting with patients who would benefit from Synapticure’s care model, and engaging with them around their needs. These conversations are a patient’s first encounter with Synapticure, after which they are connected to a Care Coordinator to develop a long-term relationship. However, until the transition to their Care Coordinator is made, you are the patient’s point person, available and eager to assist with whatever they need: support, education, or a listening ear. You work alongside, and are supported by, Synapticure’s care team to ensure patients and caregivers have the support they deserve, right from the start.
