Role Description O Profissional de Saúde da equipa CATT tem como missão efectuar o acompanhamento remoto dos pacientes da VitalAire, de acordo com o careplan estabelecido para cada terapia, fazendo uso de todas as ferramentas disponíveis (nomeadamente TechCare), contribuindo para a promoção da adesão do paciente à terapia, através de várias interações com o paciente (chamada/videochamada). - Efectuar acompanhamento remoto de pacientes, de acordo com os careplan em vigor para cada terapia. - Acompanhamento remoto de pacientes novos de sono, com realização de chamada/videochamada. - Extração, análise e encaminhamento dos dados terapêuticos dos pacientes, auxiliando os médicos prescritores no follow up do paciente. - Gerir a linha "feedback terapia" criada para facultar às equipas internas dados clínicos necessários em determinado momento da terapia, sem que haja necessidade da extração de relatório. - Gerir alertas decorrentes da terapia, mediante os fluxos definidos.
