• Act as a trusted partner to providers by managing virtual patient rooming, gathering clinical history, and preparing all necessary documentation for neurology visits. • Perform and accurately score standardized clinical assessments, including the Montreal Cognitive Assessment. • Review complex medical records, lab results, and imaging reports to identify clinical gaps and support real-time clinical decision-making. • Support patients across specialized clinic tracks, including the GUIDE Program and the Anti Amyloid Therapy program. • Assist in preparing patients for specialty therapies by coordinating required testing and obtaining supporting documentation with infusion centers and imaging facilities. • Coordinate the collection of medical records, diagnostic testing, and referrals to ensure complete continuity of care. • Facilitate crucial patient referrals for respite care and home safety assessments to support families in the home. • Recognize urgent or clinically significant issues and escalate concerns appropriately to providers or other members of the care team. • Collaborate cross functionally with physicians, advanced practice providers, care coordinators, and navigators to deliver unified, patient centered care. • Communicate compassionately with patients and caregivers while providing HIPAA compliant advocacy support across insurance, billing, and clinical hurdles.
