Role Description Drive patient outreach initiatives via phone, text, and portal messaging to proactively schedule appointments and close critical HEDIS care gaps. - Conduct meticulous medical record chart scrubbing to locate clinical evidence of care, uploading findings directly to payer portals to ensure accurate quality reporting. - Streamline daily operations and workload management using organized tracking spreadsheets to maintain high productivity and meet outreach goals. - Facilitate seamless communication with clinical practices to coordinate patient care updates, resolve inquiries, and optimize overall office workflows. - Champion process improvements by evaluating existing office functions and recommending strategic ways to simplify daily operations and cost tracking. Qualifications - High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. - Medical Assistant Training and/or experience (preferred). - Strong communication skills. - Experience with EMR/EHR and spreadsheets. - Preferred experience or knowledge of HEDIS.
