Title: Nurse Practitioner Location: Erie, PA Job Description: Employment Type: W-2 Telos Health Systems is seeking an experienced Nurse Practitioner (NP) to serve as an in-house SNF-ist, providing general medicine care to geriatric residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALF). Manage care for short-term rehab and long-term care residents. Perform comprehensive health assessments, physical exams, and diagnoses. Manage acute and chronic conditions following established protocols. Prescribe medications and treatments as appropriate. Conduct monthly stability visits for long-term residents. Evaluate residents with changes in condition. Participate in Advanced Care Planning. Collaborate with interdisciplinary teams, medical directors, and attending physicians. Complete accurate documentation and CPT coding. Facilitate referrals to specialists and other healthcare providers. What You’ll Need Required: Active, unencumbered Advanced Practice Nurse (APN/APRN) license Current DEA registration Master’s Degree in Nursing Strong communication and clinical leadership skills Ability to work effectively in a fast-paced environment Preferred: At least 1 year of experience in a Skilled Nursing or long-term care setting preferred Experience caring for geriatric population Benefits & Perks We offer a comprehensive and flexible benefits package designed to support both work-life balance and long-term professional growth. Youth Program Leaders may assist with the development of instructional content and deliver lessons and activities.      Youth Program Leaders have full responsibility for monitoring the safety, whereabouts, and actions of program participants at all times, including non-instructional times, and must be able to identify and respond to issues concerning participant interactions, behavior, mental health and acclimation to the program environment.
