Telos Health Systems staffs Psychiatric Mental Health Nurse Practitioners, PMHNPs and General Medicine Nurse Practitioners, to improve residents’ quality of life and overall well-being while simultaneously reducing return to hospital stays. What You Will Do As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents. You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Prescribe all necessary medications and treatments. Perform comprehensive physical examinations of patients. Develop and implement patient management policies and procedures. Facilitate referrals to other healthcare professionals and medical facilities. Communicate with attending physicians and/or specialists regarding patient care.
