Role Description We’re looking for compassionate and motivated Telehealth Substance Use Disorder (SUD) Counselors to deliver virtual individual and group counseling sessions that support recovery and promote wellness. - Conduct intake assessments to evaluate Member readiness and determine appropriate level of care - Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum - Provide crisis support and coordinate appropriate referrals when needed - Maintain timely, accurate documentation in our electronic medical record (EMR) system - Collaborate with clinical supervisors on individualized treatment and discharge planning - Ensure compliance with HIPAA and all relevant state regulations Qualifications - Active and verifiable Maryland counseling licenses of LCSW-C, LCPC, LCADC or LCMFT, as well as your state of residence - All credentials must be verified through the State Board’s website as part of the application process - Proficient with Microsoft Office or Google Suite, EMR systems, and telehealth platforms - Passionate about supporting individuals in recovery and building therapeutic relationships - Comfortable working both independently and as part of a collaborative team in a fast-paced, mission-driven startup environment - Have a private, HIPAA-compliant workspace and reliable internet Benefits - The salary range for this full-time position is $55,000 - $70,000 and will commensurate with experience and professional license. - Employees may enroll in offered medical, dental, and vision coverage and also provided employer paid life and disability insurance. - Employees have paid vacation, sick time, observed company holiday and parental leave. - Remote work flexibility - Opportunities for continuing education and professional development - Supportive team culture focused on impact, collaboration, and innovation - Be part of building a modern, inclusive, and accessible addiction care system - Be part of the mission. • Develop and facilitate communication and education strategies by conducting one-on-one virtual and on-site group meeting by using various technologies in a professional manner • Drive participants to take measurable actions to improve their retirement outcomes including plan enrollment, deferral increases, and rollovers from previous plans • Achieve business results and facilitate impactful conversations while meeting service level agreements • Consistently complete reporting requirements within required timeframes including logging meetings, attendees and outcomes in appropriate systems • Build strong relationships with assigned key contacts, plan sponsor representatives and internal stakeholders to maximize successful participant outcomes • Manage meeting schedule, including travel booking • Provide feedback to local relationship managers about the effectiveness of communication materials/strategies and assist in identifying alternatives • Attend client meetings with other service team members • May be asked to participate in finals presentations for new prospects based on the demands of the territory

Role Description The Disability Hub MN Options Counselor is responsible for answering inquiries and providing options counseling to individuals with disabilities. The ideal candidate will thrive in a fast-paced, high-volume environment, with strong communication skills and in-depth knowledge of local, state, and federal benefits and assistance programs for individuals with disabilities. A successful Options Counselor will manage customer interactions according to established standards and assurances. - Conduct person-centered discovery to explore the individual’s needs, strengths, values, preferences, resources, and goals. - Apply person-centered principles to provide effective counseling. - Educate clients on available public and private sector resources. - Facilitate self-direction, self-determination, and informed choice. - Ensure consumers receive the best possible support in navigating available resources. Qualifications - Must reside in Minnesota. - Lawfully authorized to work in the United States. - Employment is contingent upon successful completion of a criminal background check and education verification. - High school diploma or equivalent required; some college coursework preferred but not required.
