Role Description As an RN Maternity Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as: - Educator - Evaluator - Service coordinator - Community resource researcher The result? This is a work from home position with working with the Healthy First Steps Program. Employee must reside in the state of Florida within the Jacksonville, FL and surrounding areas. If you reside in the state of Florida, you will enjoy the flexibility to telecommute as you take on some tough challenges. Primary Responsibilities: - Making outbound calls to assess members' current health status (potential for field visits as well) - Take incoming calls from members - Identifying gaps or barriers in treatment plans - Providing patient education to assist with self-management - Interacting with Medical Directors on challenging cases - Coordinating care for members - Making referrals to outside sources - Coordinating services as needed (home health, DME, etc.) - Educating members on disease processes - Encouraging members to make healthy lifestyle changes - Documenting and tracking findings - Making 'welcome home' calls to ensure that discharged members receive the necessary services and resources Qualifications - Current and unrestricted Registered Nurse license in state of Florida - 3+ years of experience in a hospital setting, acute care, direct care experience within the OB and/or Maternity field - 1+ years of experience working with MS Word, Teams, Excel and Outlook - Reside in the Jacksonville metro area Requirements - Bachelor's Degree (preferred) - Case management experience (preferred) - Background in managing populations with complex medical or behavioral needs (preferred) - Experience with electronic charting (preferred) - Bilingual - Spanish (preferred) - Experience with arranging community resources (preferred) - Field-based work experience (preferred) - Knowledge of Florida Community Maternal Child Resources (preferred) - Demonstrated solid organizational skills (soft skills) Benefits - Paid Time Off which you start to accrue with your first pay period plus 8 Paid Holidays - Medical Plan options along with participation in a Health Spending Account or a Health Saving account - Dental, Vision, Life & AD&D Insurance along with Short-term disability and Long-Term Disability coverage - 401(k) Savings Plan, Employee Stock Purchase Plan - Education Reimbursement - Employee Discounts - Employee Assistance Program - Employee Referral Bonus Program - Voluntary Benefits (pet insurance, legal insurance, LTC Insurance, etc.)
