• Manage a clinical caseload dependent on background • Deliver education to participants via telehealth model • Carry out standardised and evidence based assessments • Work as part of a multidisciplinary team • Reporting to referrer and/or patients medical team • Take part in case conferencing as needed • Adherence to technological security measures • Adherence to all policy & procedures • Monitors delivery of care by documenting care; identifying progress toward desired care outcomes; reviewing the care plan with patients and with care providers • Respects participants and providers by recognising their rights and maintaining confidentiality; Adherence to business and technological security standards and measures
Job Requirements
- More than 4 years experience in client work in a private or community/hospital settings
- Registered with DA as an APD
- Experience working with oncology clients highly regarded
- Experience working in a variety of funding models highly regarded
- Access to stable, private internet connection
- PC or Laptop computer with camera and microphone
- Familiarity with telehealth service provision
- Excellent client centred focus along with effective communication/problem solving skills
- The ability to work independently and good time management skills
- Knowledge and understanding of various funding models, including private health insurance, occupational rehabilitation and medicare rebatable services
- Previous work experience in telehealth/digital health care would be highly considered.
- Flexibility to increase hours into the future preferable
- Demonstrated competence in the use of information technology and desktop applications; internet, word, spreadsheet, electronic medical record and database packages
