Role Description The Population Care Nurse is a registered nurse (RN) who focuses on managing and improving the health outcomes, wellness, and quality performance of specific populations or communities, particularly those with chronic conditions, high healthcare needs, or underserved groups. This role involves working within interdisciplinary teams to assess, plan, and deliver patient care at a population level, rather than focusing on individual acute care. - Implements population intervention to improve health, wellness, and quality performance. - Interventions include evidence-based preventative services such as: - Immunization - Chronic disease preventative care - Cancer screening - Medication follow-up - Wellness visits - Involved in preventative care, health education, case management, and coordination of services. - Focus on reducing hospital readmissions, managing chronic illnesses, and addressing health disparities. Role Description Provide telehealth therapy and support services for adults experiencing mental health and/or substance use challenges. You'll be part of a collaborative, multidisciplinary team while delivering services remotely via secure platforms.
