Vice President, House Calls

Role Description The Vice President of House Calls is responsible for leading one of Mosaic's most critical value-based care programs, providing in-home clinical services to high-risk and medically complex patients. House Calls serves as a key wrap-around service that extends the primary care team into the home, improving patient outcomes, reducing avoidable utilization, and ensuring patients receive the right level of support at the right time. - Drive exceptional patient care and operational performance through the management and growth of the House Calls program. - Focus on patients with advanced chronic illness, functional decline, and those requiring increased care coordination. - Work closely with primary care providers, care management, palliative care, hospice partners, and interdisciplinary teams. - Accountable for continuous improvement in operational fundamentals including: - Provider productivity - Schedule optimization - Workforce planning - Recruiting and developing clinical teams - Quality outcomes - Patient engagement - Financial performance - Play a critical role in advancing the organization's End-of-Life (EOL) strategy. - Hold full accountability for the budgeting, operations, growth, quality outcomes, and EBITDA performance of the House Calls program. Qualifications - Minimum of 5 years in a senior leadership role managing clinical teams, physicians, and APPs. - Proven experience managing full P&L operations, including budgeting and EBITDA management. Requirements - Maintain full operational and budgetary responsibility for the business unit. - Develop and execute growth strategies that expand access to House Calls services. - Monitor and improve operational performance through effective workforce planning and resource allocation. - Develop and implement clinical initiatives and evidence-based pathways. - Lead the organization's strategy for delivering in-home care to high-risk populations. - Partner with primary care providers and regional leadership. - Drive initiatives that improve quality of life and reduce avoidable emergency department visits. - Utilize risk stratification and population health data for prioritizing outreach. - Communicate provider and business unit needs to the larger MPG organization. - Work with corporate staff to maintain relationships with physicians and advanced providers. - Lead, mentor, and develop a high-performing, multi-state team. - Manage weekly leadership meetings to drive accountability and operational performance. - Ensure House Calls is fully integrated with primary care and other wrap-around services. - Leverage expertise in value-based care models to maximize quality outcomes. - Collaborate with regional and market leadership teams to educate providers on House Calls services. - Ensure strict adherence to all federal, state, payer, and organizational regulations. Travel Requirements - Travel is required as needed to support clinical teams and operations across multiple markets including Florida and California.
