• Collaborate on treatment and academic plans for patients in the REACH program • Conduct home, school, and community visits to address and support school transitions • Provide families with support and coaching while connecting them to appropriate levels of services • Work closely with patients, families, and teams to address school-related issues and transitions • Accompany patients to school if compliance is an issue • Ensure patients are receiving appropriate school accommodations • Engage outpatient mental health program patients in social and recreational activities • Articulate progress or delays in treatment to ensure a seamless transition • Ensure timely connection of patients and families to appropriate services in the Emergency Department
Job Requirements
- CYW certificate and diploma or degree from a recognized community college or university
- Knowledge and skills in DBT and CBT
- Knowledge and skills in group design, development and implementation
- 3-5 years of child and adolescent experience preferably in the hospital setting
- Knowledge of children's psycho-social development
- Able to work independently and effectively within a team
- Competency in assessing risk issues and addressing concerns within the community and schools
- Experience in working with high conflict families and skills in de-escalating conflict
- Understanding of the school system and support within the Halton community
- Computer skills (Word Processing)
