Role Description As a Telehealth Counselor, you will oversee all aspects of a patient’s treatment. You will be responsible for the coordination and continuity of care from admission through discharge and follow-up of the patients. You will also promote performance improvement and service delivery by providing comprehensive psychological therapy directed towards addressing the causes and effects of the patient’s addiction. Responsibilities - Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures. - Provide patients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of services, patient rights, and additional information to help patient adjust to the program. - Assure each patient is assessed and placed according to the treatment phases in accordance with Federal and State standards and agency policies. - Keep all patient files organized and updated for own and agency’s use in accordance with established policies and procedures. - Counsel patients in crisis situations by identifying patient resources and coping abilities to help patients cope with and take steps to resolve crisis. - Counsel patients who have refused to comply with specific program requirements to increase patient awareness of consequences of noncompliance. - Arrange referrals to other agencies and resources in the community when appropriate. - Perform treatment plan reviews. - Participate in case conferences and case reviews in accordance with established policies and procedures. - Develop and increase knowledge of federal, state, and agency regulations regarding confidentiality, methadone treatment, and substance abuse counseling certification. - Submit all internal and external correspondence to the supervisor for review and approval. - Secure necessary documentation from patients, when appropriate, to ensure compliance with established policies and procedures. - Submit monthly and quarterly statistical reports to supervisor. - Attend supervision meetings as scheduled. - Attend team meetings and complete all training courses as required. - Other duties as assigned. Qualifications - Bachelor’s degree in social science discipline and at least one (1) year substance abuse related experience. - Counselors shall be licensed or certified by the applicable Virginia Health Registry Board or by a nationally recognized certification board or CSAC eligible. - Experience with dependency and/or medication assisted treatment. • Conduct intake assessments to evaluate Member readiness and determine appropriate level of care • Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum • Provide crisis support and coordinate appropriate referrals when needed • Maintain timely, accurate documentation in our electronic medical record (EMR) system • Collaborate with clinical supervisors on individualized treatment and discharge planning • Ensure compliance with HIPAA and all relevant state regulations • Applicants must be licensed in their respective state, and all credentials must be verified through the State Board’s website as part of the application process

• Working from your secure and distraction-free home office, certified Substitute Counselors will virtually support the learning program for students in Connections Academy schools across the country. • Consult frequently and regularly with students, Learning Coaches and school personnel to ensure that each child successfully completes his/her instructional program. • Act as counselor of record at various Connections Academy schools during periods of short- or long-term absences or vacancies ranging from, but not limited to, 2-20 weeks, with the average assignment being 8-12 weeks in length. • When on assignment, be online and available to students, parents, and school staff Monday through Friday from 8:00 am to 4:00 pm in the time zone of the school to: • Communicate with parents, students and teachers on a regular basis to develop and update individualized learning plans and schedules, provide feedback on student work, suggest instructional approaches and strategies, monitor completion of assignments, and verify student learning. • Review transcripts and use those to determine appropriate student course placements per school requirements. • Support student social-emotional learning through individual and small group sessions. • Prepare students for their next steps following graduation. • Complete all grading, generate progress reports, and conduct parent conferences in a timely manner, as instructed by the school. • Support the instructional program through a combination of synchronous and asynchronous web conferencing sessions (LiveLessons). • Support students and parents with alternate instructional strategies and provide assistance with daily assignments and projects. • Keep student records and data up to date, including cumulative files, online student and family information, attendance accounting, and student logs. • When not on assignment, actively check email to remain informed of upcoming assignments and important Substitute Services updates. • Meet at least monthly with an assigned Substitute Services supervisor via video conferencing software, whether on or off assignment. • Complete required professional learning targeted at individual needs throughout the year. • Maintain active certifications.
