Role Description Chronic Care Staffing is seeking a compassionate and detail-oriented Remote Care Coordinator - LVN/LPN licensed in Illinois to support patients managing chronic health conditions through proactive outreach and ongoing care coordination. In this role, you will serve as an essential connection between patients and healthcare providers, helping improve patient outcomes through education, advocacy, and consistent communication.
