Enrollment Coordinator – CMA/RMA

• Contact patients to introduce CCM and RPM services and explain program benefits. • Guide patients through the enrollment and consent process. • Document all patient interactions and enrollment activities accurately within EMR systems. • Ensure compliance with CMS, HIPAA, and internal documentation standards. • Answer patient questions regarding care coordination services and program expectations. • Collaborate with healthcare providers and internal teams to maintain smooth communication and patient transitions. • Maintain organized workflows while managing daily outreach goals and follow-up tasks. • Support positive patient experiences through empathy, professionalism, and clear communication.
Job Requirements
- Active, nationally recognized CMA or RMA certification.
- Current Basic Life Support (BLS) certification.
- Proven experience working directly with CCM and/or RPM programs.
- Strong proficiency with EMR systems and Google Suite.
- Dedicated, HIPAA-compliant home office environment that is private and distraction-free.
- Strong time management, organizational, and independent work skills.
- Excellent verbal and written communication abilities with strong patient empathy.
- Previous experience in care coordination or transitional care management (Nice to Have).
- Strong understanding of CMS regulations, healthcare documentation, and quality reporting standards (Nice to Have).
- Bilingual fluency in English and Spanish (Nice to Have).
