Medical Records Associate

• The Medical Records Associate is responsible for obtaining patient information from healthcare professionals, categorizing conditions, treatments, and diagnoses, as well as creating permanent medical records for reference and regulatory purposes • Manage electronic health records, ensuring regulatory compliance, and supporting data-driven decision-making in healthcare settings • Retrieve medical records from healthcare practices, exam patient charts and records for accuracy, completeness, and compliance with established standards • Pinpoint any inconsistencies, missing information, or errors in the documentation • Verify the records adhere to relevant regulations, accreditation standards, and policies as applicable • Check the accuracy of patient demographics, medical history, treatment details, and other relevant information • Contacting physicians or other healthcare professionals to clarify information or resolve discrepancies • Enter and update medical records for healthcare professionals and regulatory • Update medical records in a timely manner • Ensure that patient data and medical records are protected and remain confidential • Transcribe medical data from doctors' written or spoken notes, including symptoms, possible diagnoses, prescribed medications, and tests • Transcribe patient data from eSource into sponsor EDC, if applicable • Utilize HIPAA-compliant electronic fax line to retrieve medical records • Upload and maintain IntElligo database including tracking progress and timeliness • Confirm all necessary forms and reports are present and correctly completed • Maintain the status of all patient’s medical records in the database • Adhere to IRB-approved protocol • Protect the privacy of patient information in accordance with HIPAA regulations • Perform other duties as required or assigned.

Job Requirements

  • High School diploma or GED
  • Minimum one year experience medical record reviewer role or medical assistant role
  • Minimum one year experience with transcribing medical information and performing data entry, including categorizing and coding written and spoken patient information
  • Clinical research or patient recruiting experience strongly preferred
  • Certification in health information management (e.g. RHIT, RHIA) preferred
  • Strong attention to detail and accuracy
  • Understanding and ability to utilize effective data encryption and protection practices
  • Proficiency in medical terminology
  • Excellent written and verbal communication skills
  • Strong organizational skills, including the ability to accomplish multiple tasks within the agreed-upon timeframes through effective prioritization of duties and functions in a fast-paced environment
  • Self-motivated and able to work independently
