Field Care Manager – Physical Health

• Assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state • Guide members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. • Conduct telephonic and in-person assessments to evaluate member needs • Continuously monitor member progress using regular assessments and evaluations • Develop and implement personalized care plans based on clinical judgment • Collaborate with healthcare providers, community organizations, and services to facilitate quality care • Document and submit incident reports observing organizational policies and regulatory requirements

Job Requirements

  • Unrestricted Registered Nurse license in the state of Louisiana with 2 years experience
  • Experience working with all populations (youth, adult, geriatric)
  • Knowledge of community health and social service agencies and additional community resources
  • Experience using a variety of electronic information applications/software programs including electronic medical records with excellent keyboard and web navigation skills and the ability to provide narrative documentation.
  • Intermediate to Advanced computer skills and experience with Microsoft Office
  • Ability to work with minimal supervision within the role and scope
  • Must reside in Louisiana and can work a full-time (40 hours minimum) flexible work schedule and travel to member's residence within 50 miles
