Position: My Medicine Health Pharmacy Tech III - Outpatient Department: Pharmacy HUB Services Schedule: 40 Hours- Rotating Days/Evenings, Rotating Weekend (Hybrid) POSITION SUMMARY: Under the supervision of the Outpatient Pharmacy Ambulatory Care Program Manager and My Medicine Health Pharmacist, the My Medicine Health (MMH) Technician, according to department policies and procedures, and guidelines, will work closely with providers, clinical pharmacists and the outpatient pharmacy department to provide a coordinated adherence packaged product for patients requiring additional support establishing and maintaining compliance to their medication regimens. The MMH Technician shall thrive to provide an unsurpassed patient experience by communicating to patients/providers on program specifics/details and arranging for prescription processing/filling within the outpatient pharmacy with the ultimate goal for patients to obtain an organized product they will understand allowing them to reduce or eliminate any adherence challenges. The MMH Technician shall have an assigned area and/or patient population of focus thereby providing consistent visibility and a presence to the target clients and patients. The MMH Technician shall work diligently to remove barriers to ensure prescription regimens are provided to patients. ESSENTIAL RESPONSIBILITIES / DUTIES: Responsibilities: - Present and available within assigned area to capture/support MMH patient prescription requests/needs. - Works in collaboration with patients and health care providers to identify and evaluate referred patients for My Medicine Health Program while also pursuing new areas of opportunity to increase program enrollments. - Facilitates/coordinates the process whereby prescriptions are sent into BMC outpatient pharmacy for MMH patients, processed appropriately, adjudicated to third party insurance, error resolution completed, filled and prepared for the ultimate dispensing to the patient. - Follows established MMH workflow following up with patients requesting enrollment and patients currently enrolled to initiate or continue delivery of medication regimens. - Acts as an ambassador for MMH Services by being professional, maintaining a positive and enthusiastic disposition, exhibiting excellent AIDET skills, accommodating, and knowledgeable in all patient/provider interactions. - Ensures assigned prescription processing tasks are accurate and reviewed by supervising personnel as required by law before dispensing to patients. - Motivated to meet set targets as established by Ambulatory Care Program Manager and MMH Pharmacist. - Provides feedback and/or solutions on how to improve the MMH patient/client experience/process and offer viable solutions for consideration. - Makes sound decisions and conducts necessary follow up for problematic situations that may arise during the established process. - Immediately reports to the MMH Pharmacist/Ambulatory Care Program Manager any issues or areas of concern relating to patient care or the MMH process. - Develops relationships with patients and clinic teams establishing program value as providing beneficial services and support to patients and providers. - Utilizes the patient electronic medical record to complete medication reconciliation and evaluate relevant provider notes, medication history, and any other information necessary to ensure correct medications are prepared and filled. - Initiates patient medication alignment and synchronization, patient outreach, prescription billing, filling and processing preparing for MMH Pharmacists final quality assurance. - Provides training and orientation to new team members and contributes to a professional and confidential environment at all times. - In all activities, complies with accreditation, legal, regulatory, and safety requirements. - Attends and participates in department staff development conferences or other activities related to staff development. - Develops competencies as related to position. Maintains own professional and specialized competency through continued education. - Meets hospital-wide standards in the following areas: - Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided. - Utilizes hospital’s behavioral standards as the basis for decision making and to support the hospital’s mission and goals. - Follows established hospital infection control and safety procedures. JOB REQUIREMENTS EDUCATION: - Requires a high school diploma or state accepted equivalency. CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED: - Must be registered as a Pharmacy Technician with the MA Board of Pharmacy. - Must also have national certification (CPT) from Pharmacy Tech Certification Board (PTCB) or Exam of Certified Pharmacy Techs (exCPT) EXPERIENCE: - Minimum of 4 years outpatient pharmacy as a Tech I or Tech II or equal experience required. KNOWLEDGE AND SKILLS: - Work requires effective interpersonal skills to interact appropriately with hospital staff, patients, families/visitors, physicians, colleagues, and others. - Ability to speak the English language, read, interpret and follow complicated verbal and written instruction with precision, accuracy and dependability. - Additional language skills (beyond that of English) appropriate to the patient population served is very helpful. - Ability to multi-task, prioritize essential tasks and meet deadlines. - Strong math skills required; ability to perform work accurately and pay attention to details, including being able to perform general mathematical functions (addition, subtraction, multiplication, division, percentages, ratios & proportions). - Basic computer proficiency inclusive of ability to access, enter, and interpret computerized data/information. Knowledge of Microsoft Access and Excel. - Comprehension of pharmacy law and Board regulations - Must maintain the required level of competency to perform the essential duties and responsibilities. Compensation Range: $26.92- $37.74 This range offers an estimate based on the minimum job qualifications.
