Case Coordinator

Role Description Looking to join a team coordinating care and services to support patients in their own home? We have a 6-month contract on a full-time opportunity available to work within the SA Community Care (SACC) program. In this virtual role, you’ll sit at the centre of the care journey, connecting patients, families, and providers to ensure care is coordinated, timely, and genuinely patient-centred. Key Responsibilities - Conducting clients initial phone assessment, care planning and goal setting - Coordination of in-home services to support clients receiving care at home - Manage escalations and provide support for our frontline nurses in the community - Managing consumer care throughout the consumer journey (on-boarding, triaging referrals, initial care planning within scope of practice, care plan review and discharge) - Provision of telephone support, education and advice - Discharge planning including referrals to long term services - Monitor patient progress, identify risks, and escalate concerns appropriately - Collaborate with multidisciplinary teams to ensure continuity of care - Maintain accurate clinical documentation and reporting in line with standards You will be rostered across 7-days working between 7am and 9pm – the shifts are 7am to 3pm, 10am to 6pm, 12pm to 8pm and 1pm to 9pm. Enjoy true flexibility and work from home. This role is based in Adelaide. Qualifications - Registered Nurse with current unrestricted AHPRA registration - Experience working in an acute setting (community/hospital) - Strong understanding of care coordination, case management, or complex care (highly desirable) - Digital proficiency and ability to work virtually - Excellent communication and relationship-building skills - Confident working independently while collaborating within a multidisciplinary team - Highly organised with strong clinical judgment and problem-solving skills What’s next?
