Service Coordinator Associate

Role Description This full-time, remote position supports the Service Coordinator in coordinating care for Members. The Service Coordinator Associate builds a relationship with the Members, providing the Member with ongoing telephonic outreach and assistance with service coordination needs. This position will require daylight hours and will be a remote position, however you must be located in PA or nearby states to be considered! - Promotes communication, both internally and externally, to enhance effectiveness of service coordination services (e.g., health care providers, and health care team members respectively). - Researches and assists in finding hard-to-find Members. - Supports the development and implementation of Member Care/Service Plans. - Assists in coordinating social services for Members including referrals to community organizations as requested by Service Coordinator. - Utilizes information systems to build, research and enter Member information as needed. - Works in conjunction with the Service Coordinator to initiate and revise Member Care Plans as needed. - Supports service coordination team in outreach to Members telephonically. - Ability to work day shift, weekends and holidays as needed. Qualifications - High School Diploma required. - One year of experience in Community or Public Health preferred. - Computer literacy in order to navigate through internal/external computer systems, including Excel and Microsoft Word. - Familiarity with basic medical terminology and concepts used in Care Management. - Strong customer service skills to coordinate service delivery including attention to members, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members. - Ability to effectively participate in a multi-disciplinary team including internal and external participants. - Effective communication, telephonic and organizational skills. Requirements - Internet Speed Requirements: Minimum speed is 20Mb/s download, 5Mb/s upload less than 50ms ping, and under 10ms jitter.
