Role Description Your North Star: Provide medical leadership and collaborative clinical support to a multi-disciplinary team that fosters the delivery of high-quality, evidence-based care focused on improving perinatal health outcomes to a majority Medicaid population of patients. This role reports to the Head of Clinical Solutions and is open to any candidates located in the continental US. What you'll do: - Be clinically responsible for an assigned population of patients from the perspective of individual patient care and population health metrics. - Provide collaborative physician supervision and consultation for an experienced team of APPs, RNs, registered dietitians, lactation consultants, and mental health specialists. - Lead multi-disciplinary team-based care coordination meetings and work with in-house clinical specialties (nursing, NPs, midwifery/group care, dietary/nutrition, lactation, mental health) to develop holistic individualized care plans. - Help drive our population health programs toward healthier outcomes by partnering closely with Operations leads to identify gaps and improvement opportunities, promoting our accountability culture to move the needle forward. - Partner closely with our leadership team to identify and implement interventions to address cross-disciplinary or practice performance improvement needs (e.g., updates to protocols, tech-enabled tools, trainings, policies, etc). - Facilitate and lead cross-disciplinary review sessions for episodic events and perform periodic peer review for medical providers, identifying opportunities for team education and training. - Take regular virtual call to support our 24/7 availability to patients. - Represent Pomelo’s care internally and externally.
