Social Care Manager

Role Description The Social Care Manager (SCM) works collaboratively with patients/caregivers, providers, and the Medical Management team to coordinate care. This coordination emphasizes addressing social determinants of health, conducting comprehensive psychosocial assessments, connecting members to community resources, and partnering with other agencies. The SCM assists patients, families/guardians, and the healthcare team through the care coordination process and demonstrates strong communication and interpersonal skills. Making outbound calls to enrolled members, review, research and respond to questions or issues via email or phone. This is a Monday – Friday role, 8:00 – 5:00 PM MT. You will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities - Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. - Care plan documentation, reviewing reports, identifying community resources by keeping up with updates and changes in the industry while working virtually from home. - Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services. - Manage the care plan throughout the continuum of care as a single point of contact. - Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members. - Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team. Qualifications - Bachelor’s degree in social work or Behavioral Science-related field. - 2+ years of experience in care coordination, case management, clinical or community health setting. - 1+ year of clinical experience in a healthcare setting. - Experience navigating community healthcare and social services resources. - Intermediate to Advanced proficiency using Microsoft Word (create, edit documents), Excel (create/edit spreadsheet, filter, sort, basic formula), Outlook (calendar management, emails). Requirements - 2+ years of experience in care coordination, case management, clinical or community health setting. - Background in managing populations with complex medical or behavioral needs. - Experience using Curo. - Experience with electronic charting. Telecommuting Requirements - Ability to keep all company sensitive documents secure (if applicable). - Required to have a dedicated work area established that is separated from other living areas and provides information privacy. - Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service. Pay Information The salary for this role will range from $60,200 to $107,400 annually based on full-time employment.
