• Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management. • Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques. • Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation. • Partner with the clinical care team to support patient care plans and escalate concerns appropriately. • Provide timely updates and documentation related to patient engagement and progress. • Participate in interdisciplinary team meetings. • Respond to patient communications in a timely, professional, and compassionate manner. • Build and maintain strong, trust-based relationships with patients and caregivers. • Consistently deliver a high-quality patient experience aligned with program standards.
Job Requirements
- Bachelor’s degree in health sciences or related field preferred
- NBC-HWC (National Board for Health & Wellness Coaching) certification required
- Experience in a healthcare, chronic disease management, or patient engagement setting preferred
- Cardiology or heart failure experience a plus
- Telehealth or remote patient engagement experience preferred
- Strong communication and interpersonal skills
- Ability to motivate and support behavior change in diverse patient populations
- Organized, adaptable, and comfortable managing multiple priorities
- Proficiency with electronic health records and standard computer applications
- Bilingual English/Spanish proficiency preferred

Completes other tasks assigned by manager or another member of leadership. • Completes all the necessary driver and vehicle maintenance logs, on a regular basis. • Ensure compliance with all TireHub policies and procedures, as well as Federal Motor Carrier Safety Administration (FMCSA). • Records all working hours accurately, and reviews/approves weekly timesheet by close of business on final day worked each week. • Notifies TLC Leader/AOL of any timesheet errors or discrepancies for daily correction.
