• Proactively source practice acquisition, ASC joint venture, and partnership opportunities aligned with the company’s growth strategy. • Build trust and maintain deep relationships with physician practice owners, brokers, bankers, and industry advisors. • Partner closely with physician dyad leaders to ensure alignment, credibility, and consistent communication with potential sellers. • Represent the organization externally, including traveling to target practices to present the company’s value proposition and assess cultural and strategic fit. • Develop and manage a disciplined sourcing pipeline, tracking opportunities from initial outreach through LOI. • Conduct financial and strategic assessments to prioritize opportunities and advance the most attractive transactions. • Collaborate with finance and deal execution teams to transition qualified opportunities into diligence and closing phases. • Prepare business cases, investment summaries, and pitch materials for executive and physician leadership and in-person transaction pitches with targets. • Accountable for progressing deal modeling, transaction structuring, and due diligence and related materials to into underwriting and completion. • Support transaction structuring, negotiations, and LOI development in partnership with the finance, and private equity teams. • Coordinate with internal stakeholders (finance, operations, clinical leadership, legal) to ensure progression through deal execution. • Remain engaged through signing and closing to maintain momentum and continuity with physician partners. • Maintain a strong understanding of physician practice dynamics, competitive landscape, and market trends. • Provide feedback to leadership on sourcing strategy, target profiles, and evolving market opportunities. • Continuously refine outreach strategies and messaging to improve conversion from sourced opportunity to completed transaction.

• Provide clinical oversight to a POD of Medical Assistants and Health Coaches managing a cardiac patient panel. • Ensure POD execution aligns with clinical protocols, care pathways, and value-based care objectives. • Serve as the clinical escalation point for complex cases, ensuring timely intervention and appropriate level of care. • Monitor POD performance, workload distribution, and adherence to program requirements. • Lead regular care planning huddles to prioritize enrollments, engagement, and program completion. • Collaborate with cardiology, primary care, post-acute providers, and community partners to align care plans with quality measures, risk-based contracts, and patient goals. • Deliver end-to-end clinical care management, including triage, high-risk care management, and transitional care management for complex cardiac patients. • Perform clinical triage of patient-reported symptoms, biometric alerts, and care gaps, escalating concerns per protocol to providers or emergency services as needed. • Proactively identify clinical deterioration to prevent avoidable emergency department visits and hospitalizations. • Lead high-risk care management for patients with advanced cardiac disease and multiple comorbidities through ongoing assessment and care planning. • Conduct comprehensive assessments, medication reconciliation, and individualized, goals-based care plans. • Address barriers to care such as medication adherence, social determinants of health, and access challenges in partnership with the care team. • Provide patient and caregiver education on disease management, symptom recognition, and self-management strategies. • Manage Transitional Care Management (TCM) following inpatient or post-acute discharge, including timely outreach, clinical assessment, and care coordination. • Ensure timely follow-up, medication reconciliation, and patient education to support safe transitions of care and reduce readmissions. • Document all interventions, care plans, and assessments accurately in Care Management system and the electronic medical record. • Support reporting and performance improvement related to cardiac quality metrics, program key performance indicators and utilization outcomes. • Contribute to continuous improvement initiatives aimed at improving outcomes, patient experience, and total cost of care.

• Referral Management & EHR Navigation: Proactively track down and pull missing clinical documentation required for patient referrals across multiple EHR systems (NextGen, eClinicalWorks, and AthenaOne) to ensure seamless patient onboarding. • Data & KPI Reporting: Compile, analyze, and distribute routine Key Performance Indicators (KPIs) to participating medical practices, ensuring transparency and program alignment. • Vendor Data Oversight: Review, verify, and monitor program data provided by our third-party digital health/service vendor to ensure accuracy and quality control. • Financial & Invoice Administration: Prepare and manage monthly invoices, tracking program expenditures and utilization accurately. • Physician Compliance & Documentation: Assemble, organize, and distribute monthly DocuSign packets for collaborating physicians to review, sign off on, and track.

• CVAUSA is recruiting for a Vice President of CardioNexus (CVAUSA’s Affiliate Network). • Responsible for sourcing, cultivating, and closing affiliate relationships with independent cardiology groups across target markets. • Own the full business development lifecycle from market mapping through signed affiliate agreement. • Partner with internal operations and clinical teams to ensure a strong early relationship and seamless handoff to integration. • Build and manage a robust pipeline of independent cardiology group prospects across assigned geographies, with accountability for a defined annual affiliation target. • Conduct proactive outbound outreach to practice owners, managing partners, and key physician stakeholders — by phone, in-person, and at conferences. • Develop deep market intelligence on target groups including ownership structure, payer mix, referral patterns, competitive positioning, and strategic intent. • Qualify prospects against CVAUSA's affiliation criteria and manage pipeline CRM discipline with rigorous stage-gate tracking. • Lead physician-facing discovery conversations that surface practice pain points, financial goals, and long-term succession considerations. • Collaborate with the Chief Development Officer, Chief Innovation Officer, VP of Operations and service line leads to structure affiliate economics, presenting tailored value propositions that connect practice-specific needs to CVAUSA's platform capabilities. • Drive deal progression from initial conversation through executed affiliate agreement, navigating physician partnership dynamics and legal/contracting processes. • Manage multiple complex deal tracks simultaneously with consistent follow-through. • Represent CVAUSA at cardiovascular society meetings, regional conferences, and practice management forums as a visible network ambassador. • Develop market-specific go-to-market strategies in collaboration with leadership, identifying concentrations of independent groups and referral network dynamics. • Gather competitive intelligence on other platforms, regional health systems, and PE-backed groups actively recruiting cardiologists.

• Lead onboarding of new affiliated practices, including operational transition and integration into the platform • Coordinate across internal teams (value-based care, technology, revenue cycle, clinical, and analytics) to ensure smooth implementation • Establish standardized onboarding processes, timelines and growth plans • Partner with affiliated practices to improve performance across key areas, including revenue cycle, patient access, and practice operations • Track and manage performance using defined KPIs and dashboards • Identify gaps and implement targeted improvement initiatives • Drive utilization of company services, including CVAUSA’s NSO products, revenue cycle management, care management programs, and ancillary services • Partner with practices to ensure services are effectively implemented and delivering measurable value • Monitor performance and ROI of deployed services • Serve as a primary point of contact for affiliate leadership, including physicians and administrators • Lead regular operating reviews and performance discussions • Build strong relationships and align stakeholders around shared goals • Work closely with innovation, business development, clinical leadership, operations and functional leaders to create a cohesive affiliate operation. • Provide feedback to improve platform offerings based on affiliate needs • Support development of scalable operating models across the network

• Support the administration and optimization of CTMS platforms, with a primary focus on REALTIME • Partner closely with the Senior System Administrator on system governance, enhancements, and roadmap execution • Maintain system configurations, user roles, permissions, and workflows across sites • Build and configure study calendars, visit schedules, and financial tracking tools within CTMS • Provide real-time troubleshooting and guidance to ensure smooth study execution • Develop and deliver structured onboarding and ongoing training programs for coordinators and site staff • Support the development, maintenance, and implementation of Standard Operating Procedures (SOPs) related to CTMS use, study builds, and documentation workflows
